PERSONAL COACHING AGREEMENT

Thank you for allowing Michael Bianchi, Ramsey Preferred Coach, to participate in your financial plan. I look forward to serving your needs! To help guide our relationship, I ask that you review the information below.

COACHING SERVICES

As your coach, I assume the responsibility of understanding and evaluating your current needs, goals, and resources in order to develop a strategy to meet your objectives. In order to make your plan work, I expect that you will come prepared to take the steps necessary to succeed, take full responsibility for your plan and its implementation, and contact me when you need additional coaching and direction.

PAPERWORK

You will be asked to complete preliminary paperwork so that I can be better prepared to coach you. You may also be asked to complete additional paperwork and tasks on a regular basis. I ask that you complete the paperwork and/or tasks to the very best of your knowledge and ability prior to your next scheduled appointment. You assume all responsibility for providing accurate and complete materials, documentation, and facts upon which my coaching services may be based.

SCHEDULING APPOINTMENTS

To schedule an appointment, please send an email to michael@coachmrb.comand give me 2-3 days/times that work for youand I will get you on the schedule. Please check my website, www.coachmrb.comfor my generally available times. If you need to re-schedule an appointment for any reason, please give me at least 24 hours notice.

SPOUSAL PARTICIPATION

If you are married, the ideal situation is for both spouses (or life partners) to be engaged in the process. If your spouse or life partner is not available, we will work with you as best the situation allows.

REFUND POLICY

I do not offer refunds for my coaching services as my services are “fee-for-time”. I promise that you will get out of the coaching sessions what you put into it.

CONFIDENTIALITY POLICY

I will keep completely confidential all matters discussed, facts revealed, and the nature and content of all documents you provide to me or I generate on your behalf. I will reveal such matters only upon receipt of a written authorization and clear, verifiable instructions from you telling me exactly what I may reveal; or as required by local and/or federal law.

Coaching is designed to give you accurate and authoritative information with regard to the subject matter covered. I am not authorized to render legal, accounting, tax, or other professional advice. Since your situation is fact dependent, you must additionally seek the services of an appropriate licensed legal, accounting, tax, or investment service or professional.

SERVICE FEES

Michael Bianchi, Ramsey Preferred Coach offers the following options for payment of his “fee-for-time” services. Please select the option that works best for you:

____: 3 Session package paid in advance $485

($161.67 per session, savings of $100)

____: 6 Session package paid in advance $870

($145.00 per session, savings of $300)

____: Monthly Subscription on autopay $160/mo.

(Includes 1 monthly coaching session, 1 monthly follow-up call, unlimited email support)
